Prior to your appointment, our office will send you a link to complete a series of short forms. We value your time and this process allows us to gather important information before a patient visit, streamlining the intake process.

Before your visit, please review the below information regarding Privacy Practices. New patients are encouraged to print and complete the Vasectomy Reversal Intake Form and bring it with them to their initial appointment.

All new patients should arrive 15 minutes before your scheduled appointment with Dr. Matson to process insurance and provided information, collect co-pay, and take vital signs.

Established patients should arrive 5-10 minutes prior to your scheduled appointment for check-in as well.
