Prior to your appointment, our office will send you a link to complete a series of short forms. We value your time and this process allows us to gather important information before a patient visit, streamlining the in-take process.

All new patients should arrive 15 minutes before your scheduled appointment with Dr. Matson to process insurance and provided information, collect co-pay, and take vital signs.

Established patients should arrive 5-10 minutes prior to your scheduled appointment for check-in as well.
